What are progress notes in healthcare?

Progress notes are the ultimate way to ensure effective clinical record-keeping for your practice, as they enable you to keep a record of every point of communication with your clients. Not only are they a legal requirement for accountability and tracking purposes, but progress notes are an excellent (and easy!) way to summarise your notes concisely. Any healthcare professional who reviews or assesses your clinical notes can easily understand the steps from the patient’s diagnosis to their treatment plans through its transparent format.

Not only this, but progress notes can be used in all areas of healthcare, including occupational therapy and the areas of psychology, psychiatry, physical therapy, and acupuncture. According to your practice needs, they’re a highly flexible format and are used by over 10 million US healthcare professionals daily. They serve as excellent reference points for when you need information, and they also work to improve the accuracy of your practice continually. Using progress notes is essential and vastly beneficial to the workings of your healthcare business.

Why is documentation necessary in occupational therapy?

Documentation is a vital component of every healthcare area, especially for occupational therapy (OT).

What is the purpose of progress notes in OT?

Progress notes allow occupational therapists to keep track of patient information concisely, easily understood by other occupational therapists or other healthcare professionals. It is a way for occupational therapists to note a patient’s pain, limitations, mobility, or improvements from a subjective and objective lens.

Progress notes are great as they enable practitioners to work from these observations to assess them with their judgment and evaluations, with evidence provided of exercises for the patient to complete. This then lays the foundation for the patient’s treatment.

Occupational therapists can present medical information in a transparent format to show clinical reasoning and make detailed observations with medical progress notes. And this all makes sense. We’ve all entered the healthcare space because we’re concerned about the needs of our patients, and using progress notes is the perfect way to highlight this care. With the proper documentation, we can ensure patients receive the highest level of treatment and care.

Importance of detailed progress notes for occupational therapy

To ensure your progress notes are most efficient and effective, it’s essential first to realize the significance of the letters. This way, your patients can have the highest level of care, and your practice is also highly compliant.

Provides subjective perspective - Allowing patients to voice their experience ensures that their pains and notes are heard. Part of the treatment process is working with the patient, so it’s essential to recognize the patient’s experience of symptoms, mobility, and pain.

Provides objective perspective - Detailed notes will also explore accurate observations in great length, with all relevant measurements of the patient’s exercises, treatment, pains, limitations, and improvements have taken note of. This is an excellent complement to the subjective perspective, as it allows for signs of symptoms to be shown to confirm what is being said by the patient. It provides a more scientific process, which serves as vital evidence.

Shows clinical reasoning - The more detailed your notes are, the more authoritative expertise shines through. Especially in an assessment section, providing the rationale behind all your treatment ideas will allow other healthcare professionals and providers to quickly pick up on your line of thinking. This is very significant for when the patient may need to be referred to another healthcare clinic, and they need to understand the background and reasoning of the patient’s current treatment.

Shows strategic care planning - Having detailed notes will also mean that your thought on your patient’s care will be highlighted. This is important, as your treatment plan for your patient must be highly considerate of their needs and have meaning regarding progress towards their health goals. Especially considering that treatment plans can change, having strategic planning will mean you can easily accommodate any amendments.

It didn’t happen if you didn’t write it down - Perhaps one of the most critical aspects of detailed medical progress notes is that if you didn’t write something down, it didn’t happen. Progress notes enable insurance companies to provide reimbursement for clients, and clients can track any progress. Without the evidence noted in writing, there is nothing to prove or record that an event occurred. So, you must record down essential information for safekeeping and as a reference point for when needed.

What are the critical components of occupational therapy documentation?

So you don’t miss out on any critical details, we’ve collated essential aspects, tips, and tricks to include in your occupational therapy progress notes.

How do you write an occupational therapy progress note?

To write a practical occupational therapy progress note, it is best to follow the standard SOAP format. This covers all bases for your patient and extracts the most valuable information into a concise summary.

  1. Subjective (S) - Includes the patient’s feelings and experience of symptoms and is primarily based on their firsthand account. Evidence includes direct quotes from the patient and close contacts, discussing pain level, mobility, or any limitations and improvements. E.g. “My knee pain is 4/10, and climbing stairs is somewhat difficult.”
  2. Objective (O) - Includes factually based medical information that highlights signs of symptoms. Evidence is usually data such as any exercises or treatments performed, strength or balance tests, x-rays, or vital signs. E.g., Ella has been completing partial squats (3x10 reps).
  3. Assessment (A) - Includes the synthesis of subjective and objective observations and is used to note the patient’s progress regarding their treatment plan. If it is a follow-up session, then amendments may be required to the current treatment. E.g., Because Ella’s leg stability is 7 seconds, she would benefit from working on flexion stabilization exercises.
  4. Plan (P) - This outlines the direct course of action that targets specific symptoms for the patient. The occupational therapist will work with the client towards particular goals. E.g., Ella will continue with her exercise regime but increase each exercise with an additional set of reps. She’ll also integrate stabilization exercises to improve knee mobility.

If this seems difficult to understand, don’t fret! Many healthcare businesses use therapy software for OT therapists, such as Carepatron. These platforms incorporate templates, such as this one, to ensure you don’t miss any critical details.

Tips for writing practical progress notes in OT

  1. Write legibly - If you don’t use electronic records, make sure your handwriting is neat so other healthcare professionals can read it if needed.
  2. Time is everything - It’s best to write progress notes after the session with the patient, so your consultation isn’t interrupted by constant writing. But remember not to wait too long in case you forget anything!
  3. Make every note unique - Every progress note should be specific and personalized to the individual being evaluated. General comments can make it difficult to differentiate between patients and obscure valuable insight exclusive to particular patients.
  4. Have intention - Don’t write all information said in the session just for the sake of it. Make sure that what you’re writing is relevant to the patient’s progress and their treatment.
  5. Don’t use jargon - Unclear slang, colloquialisms, spelling, or abbreviations can mean information may be misconstrued by others who read it. Universal OT terms can be abbreviated, but these should be used sparingly.
  6. Don’t be vague - Your notes should be detailed enough that any healthcare professional can pick them up and understand where your head was at.

Wrapping up

Hopefully, this has helped you consolidate your understanding of the meaning and significance of progress notes and why they are vital to the success of your practice. Not only are they a legal requirement to ensure medical compliance, but they are a brilliant way to summarize information for your patients and other healthcare professionals concisely. They show a transparent process from your patient observations and assessment to working towards an effective treatment plan.

Medical progress notes don’t have to be complicated either, with healthcare business platforms, such as Carepatron, providing built-in progress note templates. Carepatron helps support occupational therapists by helping you integrate progress notes into your clinical practice and allowing you to provide patients with the highest standard of care.

As a takeaway, progress notes are a surefire way to prioritize the needs of your patients. Incorporating them into your practice will pave the pathway for your success and significantly elevate the quality of care within your clinic - so what are you waiting for?
