What are progress notes in healthcare?

What are progress notes in healthcare?

Clinical progress notes are an excellent documentation method commonly used by healthcare professionals to show a patient’s medical history. More specifically, they allow for a paper trail of all communication between clinicians and their patients. Progress notes are a way for information to be traced, and their transparency makes them perfect reference points for teams to remember essential information. Their clear and concise structure enables the notes to be widely understood and easily interpreted by healthcare professionals, which deems them a highly recognized note-taking method.

Not only this, but using progress notes is a legal requirement for healthcare practices, in being used as evidence of medical records. If any insurance, regulatory, or law issues were to arise, keeping progress notes is a great way to hold practices accountable and to prevent such problems from occurring. Because progress notes represent every interaction between a patient and their clinician, it supplies essential information for documenting purposes.

What should be in progress notes?

While there is no right way to create progress notes, there are undoubtedly beneficial aspects to increase their effectiveness. Above all, they need to be concise and easy to understand, not to be misinterpreted by other healthcare professionals that may review the content. Progress notes are instrumental to patients’ progress towards treatment plans, and so it is essential to ensure relevant insight is highlighted in your notes.

One of the most common formats of progress notes, and arguably one of the best, is SOAP notes. This type of progress note touches on four main components that should be included:

  • Subjective - Refers to information regarding the patient’s experience and perception of symptoms, as well as their feelings. It is based on their view and often contains quotes as evidence.
  • Objective - Concerns factual information, such as medical records, examinations, x-rays, and vital signs.
  • Assessment - This is the synthesis of subjective and objective observations through interpretation and information regarding the patient’s progress.
  • Plan - Decides on a specific course of action for the patient and establishes their future goals.

Many platforms out there are designed to make progress notes for healthcare businesses, and by using embedded templates, this process of making process notes is much easier.

Tips for writing an excellent clinical progress note

While writing a progress note can seem overwhelming, we have simplified the procedure by highlighting essential features you should implement. Incorporating these tips will help shape your messages into valuable insight that can be used to develop effective treatment plans for your patients.

  • Background headers - Remember to include essential information such as the practitioner, client, time, and date on the document.
  • Use professional language - This way, information is less likely to be misinterpreted, and your notes demonstrate a level of authority. This means no jargon or abbreviations.
  • Highlight the diagnosis - This should be indicated so mental healthcare professionals are immediately aware of the issue at hand.
  • Patient appearance - It is helpful to represent the client by specifying their cognitive functioning, behavior, and demeanor at the consultation.
  • Subjectivity - You should make sure you document the client’s experience of their feelings and perception of their symptoms.
  • Safety - Any vital information about the patient’s well-being should be stated.
  • Medications - The knowledge of what the patient is being administered can provide important background information
  • Objective information - Examinations, medical records, x-rays, and relevant vital signs are essential to confirm observations.
  • Interventions - Documenting treatment currently used, such as CBT, is essential to understand the patient better.
  • Treatment plan progress - The objectives and goals of the client should always be considered, as well as their responses to treatment plans and steps.
  • Stay neutral - The best notes avoid using loaded phrases and emotive language and avoid clouding the content with preconceived biases, judgments, and assumptions.
  • Signatures - Allows for HIPAA compliance and provides accountability for healthcare practices with individual responsibility. ‍
  • Use an effective correcting procedure - Using strikethrough, as opposed to correction fluids, for mistakes means that other healthcare professionals can see the correction process. They know the original content, the error, and using initialing, they can understand who is correcting the material.

Subjective

Lisa stated she was feeling anxious for no reason, saying, “I don’t know why, but I’m always thinking about things. I can’t stop it. I can’t even sleep; I keep thinking about my life, even when nothing’s going on. It’s hard to do anything.”

Objective

Lisa demonstrates symptoms of her anxiety, with moderate to severe over-thinking of unattributable reasons. She also highlights a lack of concentration in struggling to regulate her emotions and anxious feelings towards any level of productivity.

Assessment

This is Lisa’s second session at my therapy, and she appears not to be making any progress towards alleviating her anxiety. She is currently only undertaking therapy. However, it seems Lisa would benefit from additional treatment.

Considering that Lisa is not reducing her anxiety, I will forward her to a psychiatrist to consult about potentially being prescribed anxiety medication. Lisa will continue to see me next week as we work on techniques to reduce her rumination.
